The collection kit includes everything your Healthcare Professional will need to collect your sample, including all documents and the shipping container to return the sample to us. Please ensure you have sent to us a completed copy of the storage agreement before the collection takes place. You can order the collection kit using your debit/credit card. We use the Worldpay service to ensure completely secure transactions. Alternatively you can send a cheque or complete a bank transfer for the initial payment fee.
